Attention: Choice Patients

ALL PATIENTS ARE REQUIRED TO WEAR A MASK TO EACH APPOINTMENT IF THEY HAVE ONE OR NO VACCINE

 

IF YOU HAVE HAD TWO VACCINES YOU CAN CHOOSE TO WEAR A MASK OR NOT. PLEASE HAVE PROOF OF VACCINE WHEN YOU COME TO YOUR APPOINTMENT

1)Please come alone to your appointments. 

2)Fill out your medical profile on the portal

3)Answer COVID-19 screening survey before appointments. This will be sent to you prior to your appointment, and you can reply by phone or email. 

 

The clinic and treatment rooms are sterilized often and between all patients. There is hand sanitizer available in the lobby and all treatment rooms.

When booking your next appointment or before attending your next appointment please disclose if you have recently travelled. 

​

If you have cold/flu symptoms OR have been in contact with someone with symptoms please STAY HOME, and contact your local health care provider.

If you are experiencing cough, shortness of breath, headache combined with fever or just a general feeling of unwell please cancel your appointment and contact one of the testing stations set up in the city for Covid-19.

As always, just remember to wash your hands and do not touch your face.
